Kratom is a tropical evergreen tree (Mitragyna speciosa) in the coffee family, native to Southeast Asia including Thailand, Indonesia, Malaysia, and Myanmar. Its leaves contain active compounds called alkaloids, mainly mitragynine and 7-hydroxymitragynine, which interact with receptors in the brain. For generations, laborers in the region chewed the fresh leaves or brewed them into tea. Today the dried leaf is processed into powder, capsules, and extracts and sold as a botanical product.

Common forms of kratom include loose powder (finely ground dried leaf), capsules (pre-measured powder), extracts (concentrated alkaloids as powder or resin), tinctures and liquid shots, teas, and gummies. Powder is the most versatile and economical, capsules offer convenience and avoid the bitter taste, and extracts are much stronger per gram and carry a higher risk of dependence, so they call for extra caution.

Kratom pricing depends on form, quality, and quantity. Loose powder typically costs about $10 to $20 per ounce, with bulk kilograms ranging from roughly $80 to $150. Capsules cost more per gram due to packaging and labor, usually $15 to $40 per bottle. Extracts and liquid shots are the most expensive per serving, often $10 to $30 or more each, because they are concentrated. However, its main alkaloids act as partial agonists at the same mu-opioid receptors that drugs like morphine target, so it produces some opioid-like effects including pain relief, relaxation, and the potential for dependence and withdrawal. This is why the FDA describes kratom as having opioid properties.

Kratom is sold by online vendors, specialty botanical and herbal shops, smoke and vape shops, and some convenience stores, in places where it is legal. No, kratom does not show up on standard drug tests. The common 5-panel and 10-panel workplace tests screen for substances such as THC, cocaine, amphetamines, and opiates, none of which include kratom's alkaloids, and kratom will not cause a false positive for opioids because its chemistry differs from classic opiates. Kratom is not established as safe, and the FDA has not approved it for any use. Many people use it regularly without serious harm, but documented side effects include nausea, constipation, dry mouth, dizziness, loss of appetite, and dependence. Less common but more serious risks include liver injury, seizures, rapid heart rate, and high blood pressure. Danger rises with high doses, concentrated extracts (especially products spiked with high levels of 7-hydroxymitragynine), and combining kratom with alcohol, opioids, or sedatives. Kratom withdrawal occurs in people who use it regularly and heavily, and the symptoms resemble a milder version of opioid withdrawal. Common symptoms include irritability, anxiety, restlessness, muscle aches, runny nose, sweating, insomnia, nausea, and strong cravings. For occasional users, withdrawal is usually minimal or absent. For long-term daily users and extract users it can be more intense and typically lasts from a few days up to about two weeks. Tapering the amount down gradually rather than stopping suddenly tends to reduce the severity.
